Is it only 4 slots? I was under the impression that it could be up to

You can sometimes get them from clams that drop from murlocs in the
lower level areas, more common in places like Darkshore or Azumyst

I noticed that the Winterfin murloc has one for sale in limited supply

Your assumption is correct, slot 5 = 2500g and last slot is 5000g
